What information do I need when I meet with the Funeral Director?

· Date of birth of the deceased.
· Place of birth of the deceased.
· Social Security Number of the deceased.
· Name of father.
· Name of mother (including her maiden name).
· Cemetery grave information (if applies).

What should I bring when I meet with the Funeral Director?

· Full set of clothing (including undershirt, underwear, socks/stockings, slip, bra, shoes, jewelry, etc…).
· Picture of the deceased so the Funeral Home knows how to do the hair and cosmetics.
· Objects to put in the casket.
· Military discharge (if applies) so we can get a U.S. Flag.
· Insurance policies.

What needs to be done for a funeral?

· Church and Clergy need to be contacted (the funeral home will assist in this act if the family desires).
· Prepare a eulogy.
· Decide on a memorial donation for people to make in honor of the deceased.
· Select Pallbearers.

What do I do if my love one passes away at home? Who do I call first?

· If s/he should pass away at home, you should first call the local police force. They will contact the County Medical Examiner. Once the County Medical Examiner has given the clearance for your love one to be removed by your Funeral Home, you then call the Funeral Home.

What happens with Social Security? Who notifies them?

· The Funeral Home will prepare the Social Security Form SSA 721 and send it in.

Can a funeral bill be paid by the life insurance policy of the deceased?

· Yes it can. That is if there is enough in the policy. The policy would be assigned to the Funeral Home. If the policy does not cover the total of the funeral bill, the difference will have to be paid by whomever is in charge of paying for the bill.
